Show up at Dupont Hospital for a monthly session of the Red Hot Mamas group on a recent Tuesday night, and you'll encounter a dozen 50-something women discussing "Pause and Effect: The Journey Through Menopause" with Fort Wayne psychologist Georgia Floyd. She advises them not to let the "signs and symptoms" of their midlife transition interfere with their relationships - or their sexual enjoyment."If the focus is on what you're suffering, then you can't really enjoy pleasure," she says.

They're also bringing with them attitudes and life experiences shaped in the Sexual Revolution of the 1960s - and shattering stereotypes about sex and aging as they go.


"They're not buying into it that they're too old to be sexual anymore," says Dr. Kathryn Einhaus, a 60-something Fort Wayne obstetrician-gynecologist with many older women as patients. Indeed, says Bobbie Goltz, a registered nurse who coordinates the "Red Hot Mamas" programs, women who attend generally aren't thinking that The Change means the end of sexual attractiveness or desire.


"They are looking forward to an active and healthy sex life," she says. "It wasn't too long ago that we didn't live much past menopause. Now it could be another 50 years."
Experts say if today's seniors-to-be aren't ready to relegate sex to the scrap heap, neither are their elders. Interest in sex may wane with age, and an active sex life isn't the top requirement for happiness, a major survey of midlife and older Americans by AARP found in 2004. Being in "good spirits" and staying healthy and physically active But the study of more than 1,600 people over 45 also found top the list. percent that 84 "strongly disagreed" that "sex is only for younger people." Even among those over 70, only about 6 percent of men and 7 percent of women agreed with that statement.


And, the survey found, respondents have more than opinions about sex. They also have sexual thoughts and fantasies: About 50 percent reported having them at least once a week. About 36 percent of men and 24 percent of women over 60 said they had had intercourse in the last week.

While attitudes are changing, part of the newfound stress on senior sexuality may result from the power of numbers.About 12 percent of the American population is now 65 or older. This will increase to 20% by the year 2030.


The numbers are good news in that more people such as the Davises can find new life partners, sometimes without looking. "Neither one of us was looking," Diana Davis says with a laugh. "I didn't want a relationship. My life was full and my musical career was going great, but he just kept showing up at my front door."


"Basically, seniors now are almost being inundated with information about sex, like our youth are, and feeling they need to be sexual," Harris says. "The saying is that 60 is now the new 40, like it's something that seniors are expected to live up to. ... It really has raised that bar."


Diana Davis says that, for women, sex when you're older "is different.""It's different not just because we have gone through The Change, but our desires and goals have changed. When we make love, it's because we truly are in love. It's not a fantasy or an illusion; it's love," she says."We took it very slow. We had many dinners and meals together, and we would sit in the evenings together and just talk. We became friends long before I would even consider a relationship, and it was months and months before we became intimate."
When people in the AARP study were asked to describe their partner, 76 percent said that he or she "loves me deeply."Seventy-five percent said their partner "is my friend."


Older couples even might want to wait to have sex until morning, when men's hormone levels are higher, some experts counsel. Couples might want to find positions that put less stress on joints, extend foreplay with or without intercourse, or try building desire throughout a day or weekend through verbal hints and romantic gestures.


But other adaptations mean changed thinking, says Harris, who soon will celebrate her 33rd wedding anniversary with her gynecologist husband, Lee.


"We need to broaden our definition of sexuality," she says. "It includes sensuality, touch, comfort, communication, commitment and emotional intimacy. Then we could accept sexuality more beyond the reproductive ages."


The newlywed Davises say they're already there. When Richard Davis talks about his new wife, he says, sure, she was physically attractive. But what really attracted him was that they had a lot in common. "We played together for a couple of years, and what can I say, we fell in love," he says. "It was just a feeling that we have all these common interests and I thought, wow, this could be my soul mate."

Digital certificate technology has a decidedly patchy healthcare track record. Can a new open-source model avoid the mistakes and deliver affordable, practical security?


The digital certificate was expected to allow better quality healthcare to be delivered faster and at lower cost by enabling the secure networked exchange and storage of patient and clinical data.


But it turned out to be difficult and costly to implement and the proprietary nature of many certificate solutions meant prospects for the important goal of interoperability looked bleak. As the recession bit, vendors found healthcare was among the first sectors to put certificate rollouts on hold.


So what went wrong?


I believe the basic proposition was flawed. Digital certificates are created and managed by highly complex public key infrastructure (PKI) software. The major certificate PKIs were built for the needs of global commerce - a customer base flush with cash to spend on high-profile IT projects and with well-resourced internal IT departments. The contrast with healthcare could not be starker.


It was therefore no surprise that most certificate deployments in the UK public sector took place with the help of managed service providers. In theory these third parties bear the cost of buying and operating the PKI and pass on the economies of scale. In reality, their pricing is dictated by PKI vendors anxious to recover substantial development, marketing and sales costs.


But high up-front charges are not the only problem. Most proprietary PKI needs special software to act as a kind of middleware between certificates and desktop applications. Installation and maintenance of this means further cost. Often, still more budget is sucked up by the need to integrate new and existing applications with the digital certificate service.
Finally, managed service customers have found themselves vulnerable to the fluctuating fortunes of both service providers and PKI vendors. They can be left high and dry as a result of commercial collapse or a strategic withdrawal from a particular market.


None of this means PKI is fundamentally inappropriate for healthcare. But it does, in my view, mean the established delivery mechanism is wrong.


A radical new model retains the managed services provider, but instead of being proprietary, its PKI is built entirely from industry standards-based Open Source code. The service is also configured to deliver exactly the functionality required by healthcare and to operate in a way that conforms to health sector guidelines or local requirements. It works directly with existing, unmodified applications to encrypt and sign, using no special client-side software.


Open source has its critics -- almost all of them with proprietary positions to defend -- but it is very rapidly gaining acceptance worldwide and is proving highly robust. A recent Forrester report found that 45% of sampled organisations where using Open Source in mission critical applications. The core software used by the new model, Open XPKI, is a development of teams in Italy and Germany with EU-wide governmental support. The four other main technology components: the operating system Linux; the directory service Open LDAP; the database MySQL, and the cryptographic library Open SSL, are all widely respected, deployed and proven by international business and governments.


The new PKI model starts from a uniquely strong foundation because there are no software development costs to recover. Charges therefore need only reflect the cost of service provision. Unlimited numbers of digital certificates can be issued with customers paying a simple service fee. That means a 1,000 seat project will typically cost 60% less than the old model - and it becomes cheaper still in real terms for even larger roll-outs. The new model positively encourages bold certificate deployments.


The new model is strong too in the area of compatibility. It recognises that healthcare has a huge financial and training investment in mainstream third party software and so works directly with it, enabling a wide range of secure healthcare applications to be deployed quickly and cost-effectively. Typical examples include any kind of messaging application, including electronic links with partner organisations like suppliers and other public sector agencies.


Another major plus of the new paradigm is its trust model (the hierarchy of people and organisations that make a digital certificate inherently trust-worthy). Some earlier managed services have been inflexible, insisting for example on a face-to-face verification process coupled to documentary evidence of identity. This added both delays and yet more cost.
Under the new model responsibility for verifying the identity of would-be certificate holders rests with certificate managers appointed within user organisations. They use local resources to verify applicants, and then visit a dedicated Website to order certificates.


Early experience with the new model has been encouraging. Users have found their expectations of far lower costs to be realised. Delivered solutions have proved stable, robust, and easily integrated with existing systems.

Writing all this on maternity health can be considered an obligation to us. This is because we felt obligated on imparting all this knowledge we knew about maternity health.

12 STEPS TO A HAPPY HEALTH PARROT



The right size cage with the right bar spacing and non toxic paint or a stainless steel cage.



Cage placement. This is very important. The cage should be placed where your parrot feels the most comfort. The room must have the proper lighting. Preferably natural sun light. A room that has activity. Birds are social creatures. They need to be in a flock. If you have more than one bird the other birds are part of the flock as well as all family members in the household. Parrots that are put off in a room by themselves will be very unhappy. This WILL result in screaming, biting, self mutilation, depression, poor health and can result in death. You must conceder placement very carefully. He must have the ability to interact with you and others and also be able to rest when his need arises to do so. Ten to twelve hours of uninterrupted sleep is ideal for proper rest at night.



Plenty of toys to chew on and challenge him are very important. Rotate toys weekly to keep him interested.



Time outside his cage is a must. At the minimum 4 hours a day. Longer if you can. Play stands or trees are wonderful things for parrots to have. It gives them freedom and exercise. This is important to physical and mental health.



Plenty of fresh water. Water bowls/bottles should be sterilized often and water changed several times a day. Plenty of food variety is also a must. If you feed food that is perishable make sure it is never left in his cage too long. Birds can get sick from bacteria. Parrots eat/fill their crop twice a day. Morning and evening. That is when fresh food should be provided. Make meal time a family affair. Like I said birds are flock creatures. They will want to eat when you eat.



Interact with your bird. You don't have to pick him up to give him attention. Just talk to him. He will talk back.



Excessive screaming is a sign of an unhappy and or stressed bird. All birds scream some. It's their way of communication. Your job is to learn what they are saying to you. Sometimes they scream because they feel great and want to share with everyone how good they feel. Sometimes they scream to warn of danger or fright. Sometimes they scream because that is what they have learned from other members in the house. The louder you are the louder your bird will be.



Vet exams on a regular basis are also important. Have a full blood panel done on your first visit to the vet. This will allow your vet a base line on your bird's health for the future. If he ever gets sick it will help in diagnosing a problem faster. Learn what your bird's droppings should look like and observe them on a daily basis. If an illness arises their droppings are the first clue.



Keep all toxins away from your bird. Basically this means you must bird proof your house. Just like child proofing in a way. Birds are affected by toxins in the air as well as toxins in the items they chew. So make yourself aware of what he can come in contact with.



Bathing on a regular basis is also important. All birds need to bathe and enjoy it very much. There are many ways to bathe your parrot. Taking him in the shower is an easy way to bathe him and if he tolerates it can be an important bonding time. Bathing time is also a good time to socialize him to a family member that is having trouble handling him. Most birds pick one person to bond with. This can be a problem when the need arises for someone else to care for their needs. So it's a good idea to get your bird used to having someone else fill his needs every once in a while.



Establish you're self as the flock leader as soon as you bring your parrot home. This is very important! He must know to respect you to avoid aggression throughout his life. The best way to do this is to practice the step up/ step down command.


12. Keeping your birds wings clipped is also suggested by this author. Parrots that are allowed to fly free indoors are an accident waiting to happen. Keeping him safely on his cage or stand/tree is more difficult if he can fly anywhere he wants. Keeping him where he is safe is your goal. Anyone can learn to clip wings properly. If you have established yourself as flock leader and have a good relationship with your bird you can learn to clip wings without any trauma to him what so ever. Of course have your vet show you how the first time. Nail trimming can be more difficult. And dangerous if not done right. The proper perches can limit the frequency of trimming. If you do decide to do your birds nails yourself learn how to safely and have the proper equipment on hand. A bird can bleed to death from a nail clipped to short if the bleeding isn't stopped. So make sure you know what you are doing before you attempt to nail trim. Beak trim really should not be necessary if your bird is healthy and has the right environment to keep his beak trimmed down himself. If you notice his beak over growing take him to the vet and have it trimmed. And then access why it became over grown in the first place.
